In Memoriam Giving is
a way which you may prefer to remember and celebrate the life of
a loved one, whilst helping the Myositis Support Group.
Many people prefer to pay tribute to a loved
relative or friend by a charitable donation instead of flowers.
Your funeral director will be able to make the necessary arrangements
to receive donations and forward them on to the Myositis Support
Group.
As this can be a distressful time the donations
are typically not given with Gift Aid Declaration. At a time of
sadness it would be inappropriate to ask after a donation has been
made for a Gift Aid Declaration form to be completed. However, for
this reason we have produced a Memoriam Donation Form which you
may wish to use. This form also describes the illnesses in brief
and the work of the Myositis Support Group. It is available to be
downloaded here
but if you prefer we can send them to you in the post.
A personalised Memoriam Donation page can also
be created as a tribute and to receive online donations at our secure
Just
Giving site.
All gifts given in memoriam are recorded by the
charity and any specific requests are endeavoured to be filled.
These gifts have enabled us to continue our work of improving the
diagnosis, treatment, management and understanding of Myositis in
the hope of a cure.
All correspondences should be addressed to:
Myositis Support Group,
146 Newtown Road,
Woolston,
Southampton,
Hampshire,
SO19 9HR.
Cheques should be made in pounds sterling and
payable to the Myositis Support Group.
On receipt of Memoriam Donations we will formally
send acknowledgement to the next of kin and any further persons
who have requested us to do so.
Please contact
us if you have any questions about in Memoriam Giving.
