In Memoriam Giving is a way which you may prefer to remember and celebrate the life of a loved one, whilst helping Myositis UK.

Many people prefer to pay tribute to a loved relative or friend by a charitable donation instead of flowers. Your funeral director will be able to make the necessary arrangements to receive donations and forward them on to Myositis UK.

As this can be a distressful time the donations are typically not given with Gift Aid Declaration. At a time of sadness it would be inappropriate to ask after a donation has been made for a Gift Aid Declaration form to be completed. However, for this reason we have produced a Memoriam Donation Form which you may wish to use. This form also describes the illnesses in brief and the work of the Myositis UK. This form can be printed and sent to you or can be emailed upon request.

A personalised Memoriam Donation page can also be created as a tribute and to receive online donations on the secure Giving Websites Just Giving and Virgin Money Giving.

All gifts given in memoriam are recorded by the charity and any specific requests are endeavoured to be filled. These gifts have enabled the charity to continue its work of improving the diagnosis, treatment, management and understanding of Myositis in the hope of a cure.

Cheques should be made in pounds sterling and payable to Myositis UK.

On receipt of Memoriam Donations formal acknowledgement is sent to the next of kin and any further persons who have requested.

GIFT AID - Make your donation worth almost a 25% MORE at no extra cost to you
If you are a UK taxpayer and you instruct us, we can reclaim the tax you've paid on your donation and your donation will be worth more to Myositis UK.

For every £1 you donate we can reclaim can extra 25p
A £10 donation once the tax is reclaimed is worth £12.50
A £20 donation once the tax is reclaimed is worth £25
A £50 donation once the tax is reclaimed is worth £62.50


For us to do this you must declare that you wish for your donation to be reclaimed under the Gift Aid scheme.

The additional money is reclaimed from the Inland Revenue and does not cost you anymore than the original donation you made.

For your donation to be processed under the Gift Aid scheme you (the donor) must have paid enough UK Income or Capital Gains Tax to cover all of your charitable donations (other taxes such as Council Tax and VAT do not qualify) in the year.

If you are donating on either of our secure online servers there is a box for you to tick.

If you are sending a donation by cheque please include a note to request a Gift Aid declaration form and we will return one to you for completion.
