If you wish to continue your free membership of Myositis UK, please ensure you return your membership update form by 5 April 2021.
These were sent to you either my email or post (depending on your preferences) back in February. A reminder was also sent out in March.
As we do not have annual membership fees, completing this form is our way of making sure that the membership is up to date.
If you have lost your form, please download this one.
Please email your completed form to msg@myositis.org.uk or post directly to the office.
If for any reason you cannot download the form, please email us the information.
If we do not hear from you by the 5 April 2021 we will assume you no longer wish to be a member of Myositis UK and your details will be deleted.
